Osteoarthritis is a painful degenerative condition of the joints.

The knee is one of the joints most affected, and knee osteoarthritis can be excruciatingly painful.

By the age of 60, almost 1 in 8 people suffer with osteoarthritis of the knee.

Treatment is usually limited to anti-inflammatory treatments – many of which can have pretty severe side-effects.

But like many chronic degenerative diseases, knee osteoarthritis is caused by low metabolism.

Some wavelengths of light can increase the metabolic rate locally.

Researchers have found that light therapy can treat pain and disability caused by knee osteoarthritis.

This excellent review of human studies was carried out at the University of Bergen in Norway. The results were published in the British Medical Journal.

Research into this type of treatment has been going on since the 1960s – mostly in Eastern Europe.

This light therapy can be very effective – but only when it is dosed properly.

Because of the critical importance of dosing, there are many contradictory results in the scientific literature.

So this study was designed to separate the wheat from the chaff.

The researchers looked at 22 previously completed human studies.

They analyzed these studies, looking for the effect of proper dosing on outcomes of pain and disability in knee osteoarthritis.

“We investigated whether a low level laser therapy dose response relationship exists in knee osteoarthritis.”

Low level laser therapy decreased knee pain almost immediately.

“Pain was significantly reduced by low level laser therapy compared with placebo at the end of the therapy session.”

There was also a medium-term painkilling effect from the light therapy – knee pain in general was decreased 1 to 12 weeks after beginning treatment.

Then the researchers looked at only the studies which had used optimal light dosing parameters.

Properly dosed low level laser therapy was even more effective at reducing pain.

“Pain was significantly reduced by the recommended doses compared with placebo at the end of the therapy and during follow-ups.”

The research encompassed data from over 1,000 participants.

The analysis found that the light treatment significantly reduced disability caused by knee osteoarthritis.

”Disability was also statistically significantly reduced by low level laser therapy.”

These types of light treatments have a “biphasic dose response” – meaning that too low or too high of a dose are both ineffective.

These days there are many different red and near infrared therapy lights on the market.

While these devices can be useful, they are usually sold without any care to provide proper dosing instructions…

The research in question found specific dose recommendations from the studies that they analyzed.

“Low level laser therapy reduces pain and disability in knee osteoarthritis at 4 to 8 J 4-8 J with 785-860 nm wavelength and at 1-3 J with 904 nm wavelength per treatment spot.”

Dosing light therapy can be quite confusing and technical for people not familiar with these types of parameters.

but this type of therapy is becoming more widespread among physiotherapists and other physical therapists.
