There have been a lot of important discoveries about heart failure made in the last 10 years.

Nutrients like vitamins and magnesium have turned out to be very important for those at risk of heart failure.

Researchers have also discovered a strong link between hormones and heart failure.

Particularly thyroid hormones and testosterone (in men).

Men with low testosterone have a greater risk of developing heart failure.

Men with heart failure and low testosterone have poor outcomes.

Scientists recently investigated the relationship between low testosterone and survival times.

They specifically looked at men who had been fitted with a device to replace the heart function.

Men with low testosterone had far worse survival rates after surgery to implant a ventricular assist device.

A ventricular assist device performs some of the functions of a weak heart.

The research was carried out at the Ege University Faculty of Medicine, Department of Cardiology, Izmir, Turkey. The results were published in Translational Proceedings.

By now, the relationship between low testosterone and risk of heart failure is very clear.

“Testosterone deficiency is associated with mortality in patients with heart failure.”

One treatment for heart failure is the partial or complete replacement of the heart with a ventricular assist device.

Before this study, there was no research looking at the effect of low testosterone on survival rates of men with these devices implanted.

“The effects of testosterone deficiency on patients undergoing Left Ventricular Assist Device (LVAD) implantation are unclear.”

So the researchers looked at total testosterone and free testosterone in men who had undergone these procedures.

“We investigated the role of total testosterone and free testosterone levels on mortality undergoing left ventricular assist device implantation.”

The researchers analyzed data from over 100 men.

These men all had testosterone levels tested in the month prior to their operation to implant the heart replacement device.

“101 consecutive male patients who underwent implantation and had plasma free and total testosterone levels measured in the last month before operation were included in the study.”

The researchers then looked at how the men did after the surgery. They looked at a number of factors including mortality.

“Demographics, follow-up, and mortality data were analyzed retrospectively.”

Judging by total testosterone, one third of the men had clinically low testosterone levels.

Going by free testosterone – almost 2/3 of the men had low T at the time of surgery.

“Total testosterone was below normal range in 32% of men. Free testosterone was below range in 63% of the men.”

Men with low testosterone had far worse survival rates according to either measure of testosterone levels.

“Cumulative survival rates were significantly worse in patients with low total testosterone or free testosterone than patients in the normal range.”

Those with low total testosterone fared the worst.

Those with normal levels of free testosterone fared better.

Men with free testosterone levels in the normal range had vastly greater chances of survival at 1 and 4 years after the surgery to implant the device.

The researchers then performed another analysis.

In this analysis they included other risk factors like age, cholesterol, kidney disease, diabetes, and inflammation.

Even with those risk factors accounted for, low testosterone was associated with a significant risk of mortality after the surgery.

“Further analysis after adjustment for important clinical variables, showed that low testosterone was independently associated with poor survival.”

Men with low testosterone were almost 4 times more likely to die during the follow-up period in that analysis.

The research further supports the importance of optimizing testosterone levels in men with heart failure.

Testosterone supplementation should be overseen by a well-trained medical professional.

Taking inappropriate amounts of testosterone can cause even more problems than low testosterone.

“Low total testosterone and low free testosterone levels are independent risk factors for mortality in patients with left ventricular assist devices.”
