Chronic obstructive pulmonary disease (COPD) is a broad term for potentially serious lung conditions…
Conditions which are caused by things like smoking, air pollution, fumes and chemicals.
COPD is the third leading cause of death worldwide – leading to millions of deaths every year.
Symptoms include chronic progressive respiratory problems, difficulty in breathing, chronic coughing and phlegm production.
Recent research shows that some of the aggravating factors for COPD are opportunistic infections.
When the lungs are weak and inflamed, otherwise harmless bacteria can trigger severe bouts of COPD.
And we now have evidence that bacteria from the mouth are a major trigger for COPD exacerbations.
The good news is that advanced dental cleaning significantly reduces COPD attacks.
The human research was carried out at Örebro and Lund Universities in Sweden. The paper was published in the International Journal of Chronic Obstructive Pulmonary Disease.
By now, most people are aware of the importance of gut bacteria.
But this bacterial microbiome stretches right up into the mouth too.
In the last 10 years we have learned a lot about health problems like cardiovascular disease and dementia…
Both of which can be caused by bacteria originating in the mouth.
These mouth bacteria can also cause lung problems.
“Infections from the oral microbiome may lead to exacerbations of chronic obstructive pulmonary disease (COPD).”
The Swedish researchers wondered whether dental cleaning to remove pathogenic bacteria could have any effect on COPD.
So they designed a study to try to answer that question.
“We investigated whether advanced dental cleaning could reduce exacerbation frequency.”
They looked not only at the frequency of COPD attacks – but also about the types of bacteria inhabiting the mouth.
“Secondary outcomes were lung function, and whether the bacterial load and composition of plaque microbiome at baseline were associated with a difference in outcomes.”
100 people with COPD were recruited for the study. They were randomly split into 2 groups, and everyone was assigned dental appointments.
Half of the people had a dental assessment.
The other people had a dental assessment and advanced dental cleaning.
“At baseline and at 12 months, data of exacerbations, lung function, COPD Assessment Test, and periodontal status were collected from questionnaires, record review, and periodontal examination.”
The researchers found that advanced dental cleaning every 6 months significantly reduced COPD attacks in those with regular problems.
“The annual exacerbation frequency was significantly reduced in patients previously experiencing frequent exacerbations and in those with repeated advanced dental cleaning compared with the non-treated control group.”
More advanced statistical analysis seemed to confirm the beneficial effect of advanced dental cleaning on the rate of future COPD attacks.
“The result was confirmed in multivariable linear regression, where the risk of new exacerbations was significantly lower in patients with repeated dental cleaning.”
The analysis of mouth bacteria also showed a positive result.
They were able to show that the existence of certain patterns of bacteria did increase the risk of health problems related to COPD.
“The composition of microbiome at baseline was moderately predictive of an increased risk of worsened health status at 12 months.”
The research shows that mouth bacteria play a significant role in chronic obstructive pulmonary disease.
It may not be the initial cause in every case but it is a common trigger for attacks.
The study also shows that advanced dental cleaning has a significant effect on these bacteria, reducing future COPD attacks.
“Advanced dental cleaning is associated with a reduced frequency of COPD exacerbations. Regular periodontal examination and dental cleaning may be of clinical importance to prevent COPD exacerbations.”
