Is this a safe way to treat pain?
With many doctors afraid of recommending too many opioids, and federal guidelines cracking down on opioid Rx guidelines…
….doctors are looking to other treatments for pain management.
Some of the treatments they are turning to is called Gabapentin and Lyrica.
These are treatments that have been traditionally used to treat nerve pain.
Gabapentin (Neurontin) and pregabalin (Lyrica) are both used to treat nerve pain.
Now however, they are being more broadly recommended for general pain…
…without a lot of testing to see how they work and what the side effects will be for this type of pain.
These treatments may be problematic and not as well understood as they should be, especially considering the amount they are given.
There are over 18 million scripts out there for this class of treatments.
Now gabapentin (Neurontin) and pregabalin (Lyrica) are on the ascendency and opioids are shunned. At last count, nearly 9 million people filled more than 44 million ‘scripts’ for gabapentin. Another 9 million ‘scripts’ were dispensed for pregabalin.
The number of recommendations for these treatments keeps going up, even though cases of nerve pain remain relatively stable.
The use of gabapentinoids more than tripled between 2002 and 2015.
One of the big reasons that this is happening is because medical and pharmacy schools don’t teach doctors and pharmacists much about pain management.
That means that they have to guess a lot and go with guidance from Big Pharma – which is very profit motivated.
It’s a recipe for problems! Especially when there aren’t a whole lot of good pharmaceutical solutions for pain.
One of the places that the nerve treatment Gabapentin is being recommended more frequently is after surgeries.
“Gabapentin has been increasingly used as part of a multimodal analgesia regimen to reduce opioid use in perioperative pain management. However, the safety of perioperative gabapentin use among older patients remains uncertain.
“Question: Is perioperative gabapentin use associated with in-hospital adverse clinical events among older adults after major surgery?”
But the medical community should probably be asking questions about whether or not recommending this treatment is a good idea.
It has some serious side effects that are important to be aware of.
Some of the concerning side effects of this treatment are dizziness and delirium. Pneumonia is another serious side effect.
In this cohort study, perioperative gabapentin use was associated with increased risk of delirium, new antipsychotic use, and pneumonia among older patients after major surgery. These results suggest careful risk-benefit assessment before ‘recommending’ gabapentin for perioperative pain management.
This is particularly troubling for older patients where dizziness can cause falls and delirium can be mistaken for cognitive impairment.
Pneumonia can be deadly in vulnerable populations.
Ultimately, pain management can be tricky.
Especially if you have chronic pain that has become embedded in your neuropathways.
You have to make the decision for pain management that is best for you.
But it’s important to understand the risks that you are taking with specific treatments…
…and to be able to make decisions based not only on the benefit but also on potential problems that can occur.
You should always pay attention to pain, especially if it’s new.
