Cluster headaches have a reputation for being one of the most painful chronic health problems.

They are also notoriously difficult to treat.

Cluster headaches come on very suddenly and produce a piercing and intense pain which can last for hours.

The relatively rare condition is so excruciating that it has been linked to numerous suicides.

If you look at support groups for cluster headaches you will see that many otherwise law-abiding and sober citizens have turned to illegal treatments…

Treatments like low-dose LSD and magic mushrooms.

You see, conventional medical treatments have very limited effect on cluster headaches.

But some recent research indicates that high-dose vitamin B1 could provide significant relief for some people who suffer with cluster headaches.

The case report comes to us from Università Cattolica di Roma, Largo Agostino Gemelli, Roma, Italy. It was published in Case Reports in Neurological Medicine.

Cluster headaches are a relatively unknown quantity – researchers and doctors really are not sure what causes them.

But a couple of clues have been found in the last 10 years.

One region of the brain, the hypothalamus, seems to play some role in the development of cluster headaches.

And cluster headache sufferers tend to have problems with sugar metabolism.

“The hypothalamus is involved in the pathophysiology of cluster headache. Some authors have found that patients affected by this disease have abnormalities in glucose metabolism.”

A number of things are indicated for these types of sugar metabolism issues – one is vitamin B1, also known as thiamine.

Some people don’t get enough thiamine – but we are learning that some people also have problems with thiamine metabolism.

Those folks need a heck of a lot more thiamine in order to feel the beneficial effects of the vitamin.

Some doctors decided to see what the effect of high-dose thiamine is in a patient with untreatable cluster headaches.

“Considering the role of thiamine in brain function, energetic metabolism, and pain modulation, we treated a patient affected by cluster headache with oral high-dose thiamine.”

The 41-year-old man had been suffering from cluster headaches for 25 years.

The researchers started him on daily high-dose thiamine supplements.

The researchers started him on 250 mg of thiamine once a day.

Every 3 days they increased the dose by another 250mg until the man was taking a total of 750mg per day.

This is far more thiamine than is needed to cure standard thiamine deficiency.

The patient cluster headaches were tracked using a standardized questionnaire called the Cluster Headache Quality-Of-Life Scale.

This was done before starting thiamine therapy, and again 1 month later.

The patient reported rapid resolution of the cluster headaches.

“The patient had a progressive improvement in symptoms. Headaches decreased in frequency and disappeared completely within 10 days.

The man had suffered almost daily cluster headaches at 4 AM for 25 years.

This was completely resolved by 750mg of vitamin B1 in 10 days.

Something else interesting occurred. The researchers upped his dose, and this led to the recurrence of his cluster headaches.

“The patient reported short-term episodes described as “a weight on the head” and the dose was increased to 1000 mg per day. This caused a recurrence of a headache episode of the same pattern.”

When the dose was lowered back down to 750mg per day, headaches disappeared again.

“The dose reduction to 750 mg a day caused the complete absence of headache attacks.”

This sort of experimentation with dose is something you rarely see in clinical trials.

And it seems to be a critically important factor in the treatment of this man’s cluster headaches.

The amount of thiamine needed to resolve the cluster headache may vary quite a lot from individual to individual.

A few months after starting treatment, the researchers wanted to take him off thiamine therapy to see if the cluster headaches would return.

The man was unwilling to do so – and who could blame him?

A month after that the experiment happened by accident. The man went on vacation and forgot to bring his vitamins.

“In May 2017, the patient forgot to bring his timing during a short vacation. Approximately 48 hours after the last dose of thiamine, he had a typical cluster headache attack lasting about 60 minutes.

Thiamine (vitamin B1) has almost no known side effects.

And in the last few years, a number of tricky conditions like chronic fatigue syndrome and fibromyalgia…

…have shown some responsiveness to high-dose thiamine treatment.

It’s believed that a small but significant number of people have problems with transporting thiamine to certain parts of the body.

This can be relieved by simply increasing the amount of thiamine circulation.
