The human research was carried out at University Hospital RWTH Aachen in Germany. This paper was published in Clinical Gastroenterology and Hepatology.

More and more research shows a strong relationship between things that leak from the gut and liver disease.

“Growing evidence supports the role of gut-derived metabolites in nonalcoholic fatty liver disease (NAFLD).”

When the gut becomes leaky, we end up with elevated endotoxin levels in the blood.

The authors of this study wanted to look at the relationship between these bacterial byproducts and nonalcoholic fatty liver disease.

“The relation of endotoxin levels with gut permeability and NAFLD stage remains unclear. This systematic review with meta-analysis aims to provide further insights.”

The authors looked for previously existing studies that quantified the amended endotoxin in the blood of patients with nonalcoholic fatty liver disease.

They wanted to analyze these papers, to combine a lot of data looking at the relationship between endotoxin levels and the severity of liver disease.

Ultimately, they settled on 34 high-quality human studies for their analysis.

Compared to healthy people, those with liver problems had higher blood endotoxin levels.

“Blood endotoxin levels were higher in patients with simple steatosis vs liver-healthy controls.”

Higher endotoxin levels were associated with more advanced liver problems.

“Blood endotoxin levels were higher in patients with nonalcoholic steatohepatitis vs patients with nonalcoholic fatty liver.”

The same is true when looking at the physical characteristics of the disease livers in the lab.

Leakier gut = more endotoxin in the blood = more advanced liver problems.

“Consistently, higher endotoxin levels were observed in patients with more advanced histopathological gradings of liver steatosis and fibrosis.”

The researchers also found higher endotoxin levels were associated with increased weight gain.

Previous studies show clear causal links between endotoxin, obesity, and type II diabetes.

“An increase of blood endotoxin levels was partially attributed to a body mass index rise in patients with NAFLD compared with controls.”

Endotoxin was a significant predictor of liver disease and severity even after accounting for all other factors.

Unsurprisingly, endotoxin was also associated with increased inflammation and gut leakiness.

“Increases in blood endotoxin levels were associated with increases in C-reactive protein concentrations, and in most cases, paralleled a rise in markers for intestinal permeability.”

The author suggests that blood endotoxin labs could be used to ascertain the extent of nonalcoholic fatty liver disease.

“Our results support blood endotoxin levels as a relevant diagnostic biomarker for NAFLD.”

Blood endotoxin levels are rarely tested – but studies like this show just how important endotoxin is in causing chronic and metabolic diseases.

“Blood endotoxin is a marker both for disease detection as well as staging during disease progression, and might serve as a surrogate marker of enhanced intestinal permeability in NAFLD.”
